| Provider Participation Enhancement

ChoiceCare
prides itself on listening to client needs when
establishing business in geographic locations and
incorporating provider involvement.
NCC
identifies prospective providers
through the following methods:

Client
Payment History:
Clients may provide NCC
a history for the previous
year or year-to-date of
provider payment. This
information is submitted
to NCC's
Provider Relations
Department. Providers that
are currently not participating with NCC will be identified
to begin or update
contracting efforts.
Clients may want to
establish a dollar amount
that would initiate the
contracting process. For
example, non-participating
providers with a payout of
$1,000 or more should be
contracted.

Client
Request to Contract
Provider:
Clients may have specific
providers they would like
to have participate with NCC,
such as those with
excellent reputations in
the community. NCC
will provide all clients a
"Request to Contract
Provider Form", which
clients complete and
forward to our Provider
Relations Department.
Clients are updated with
regard to the status of
our contracting efforts in
response to their special
request.
